Registration Procedures
2010-2011
Please find the necessary forms for registering your child at Our Lady of Fatima School. All forms must be completed with the necessary information prior to the interview. Children will not be considered for admission to Our Lady of Fatima School unless all information is made available to the administration at the time of the interview.
Students who wish to enroll in Our Lady of Fatima School are accepted based on the following priority established by the Diocese of Wilmington.
1. Catholic registered members in good standing of Our Lady of Fatima Parish with siblings in the school
2. Catholic registered members in good standing of Our Lady of Fatima Parish
3. Siblings of other children attending Our Lady of Fatima School
4. Catholic non-parishioners
5. Non-Catholics
**In order to attend Kindergarten, a child must be 5 years old on or before August 31, 2010.
All families seeking admission to Our Lady of Fatima School must schedule an interview with the Principal.
For all registrations, the following information must be brought to the interview:
· Birth Certificate (not hospital record)
· Baptismal Certificate (2 copies)
· Record of Immunizations
· Social Security Number
· Completed Registration Form
· Completed Religious Education Form
· Completed Parish Verification Form
· Grades 1-6 - Most recent and previous year report cards
· Grades 1-6 - Most recent standardized test results
· Grades 1-6 - Letter of recommendation from present school
· Letter of recommendation from Pastor (Required for any family not registered at OLF)
· All additional paperwork included in this packet
Determination of Tuition Rate
We naturally feel an obligation in justice to those who are registered, participating, and contributing members of our parish. They are given preference in being accepted into Our Lady of Fatima School. In checking to see if a family has been contributing to the parish, we rely, of course, on the record indicated by their use of weekly Offertory Envelopes. The “reduced” tuition rate is given only to those who have been faithfully using their envelopes for their offering for a year.
A $225.00 non-refundable Family Registration Fee is required.
Parents will be notified by mail whether or not their child has been accepted to Our Lady of Fatima School. Upon acceptance into Our Lady of Fatima School, information regarding tuition will be sent directly from the parish office. One (1) months’ tuition and completion of the FACTS Tuition Forms must be completed within 10 days to insure placement. Tuition is payable over 12 months (June through May). The June tuition is not refundable.
Parents and Guardians are advised at this time
that space is limited in all grades.
